Intrauterine Insemination (IUI) Treatment

IUI treatment is one of the most common among fertility treatments. It involves placing the prepared sperm from the male partner, into the uterus by means of a small plastic catheter. This procedure of insemination is timed to coincide with the release of the egg so as to increase the chances of pregnancy.

IUI treatment is most suitable for couples having unexplained infertility. Unexplained subfertility is confirmed if the couple are unsuccessful in achieving pregnancy and all the standard tests are normal.

Indications for IUI

  • Unexplained subfertility
  • Mild abnormalities in the sperm parameters such as slightly low count, low motility and morphology
  • Mild Endometriosis
  • Coital Difficulties
  • HIV, Hepatitis B/C Discordant couples

To address the above fertility issues, IUI is performed

Step 1

Ovarian stimulation

In an IUI cycle, clomiphene tablets or letrozole  or hormone injections are given to stimulate the ovaries. This helps in the production of more than one egg, to improve the chances of pregnancy with IUI.

Step 2

Monitoring egg growth with ultrasound

We monitor each follicle, which is a small fluid-filled sac containing the egg. We check for its growth in response to the medication through scans. These are internal (trans vaginal) scans. In one treatment cycle, you will need three or four scans.

Step 3

Egg Release

When 1 – 3 follicles reach the mature size of more than 18 mm in diameter, HCG injection is administered, to facilitate ovulation approximately 36 hours later.

Insemination is usually performed approximately 24-36 hours after the HCG injection. The healthy and best swimming sperm are separated from the semen by special preparation techniques for artificial insemination. The sperm thus prepared are kept in the incubator until they are ready to be inseminated.

The procedure of insemination is not at all painful and takes just about 5 minutes. The sperm is inserted well within the womb with the help of a fine soft tube called a catheter. You are advised to lie down or 15 minutes following the procedure.

Follow Up

You will be advised to attend the clinic after two weeks following the IUI procedure, for a urine pregnancy test. If your test for pregnancy is positive we advise you to visit two weeks later for an ultrasound. However, if the test is negative, the next treatment cycle can be started from the 2nd day of the period. One needs to give this procedure around three cycles, if still there has been no pregnancy, then one may proceed for further treatment options.

In general, with one cycle of IUI the pregnancy rate is around 20%. Upto 6 cycles of IUI could be tried before moving to IVF. However, most of the pregnancies resulting from IUI occur within the first 3-4 cycles of IUI treatment.

Success rates of IUI

Watch Dr Vyjayanthi our fertility specialist explain the success rates of IUI or IVF. The rate of success in one cycle of IUI is 20-25%. The patient can be offered up to a maximum of 6 cycles. Majority of the pregnancies happen in the first 3 cycles. If at the end of 3 cycles, pregnancy does not happen, further options like Laparoscopic key-hole surgery followed by 3 more cycles of IUI or IVF are discussed.
